MichaelSMITH(JAKE) December 10th, 2014. Peacefully in the care of Leighton Hospital, Jake passed away aged 75 years. The beloved husband of Dorothy, dearest dad to Nicholas and Michael. Jake was also a cherished grandad who will be sadly missed and fondly remembered by all his family and friends. Jake's Funeral will take place on Thursday December 18th at the Crewe Crematorium Chapel at 11.20 a.m. Donations are welcome to Macmillan Cancer or Cancer Research.
