JakePATCHETTAged 23, passed away tragically on 19th October 2018. Treasured son to Kim and Richard and loving brother of Chloe, Emily, Zak and fun uncle to Mason. Service to be held at Chanterlands Crematorium, Large Chapel on Tuesday 6th November at 1.30pm. Family flowers only please, although donations would be gratefully received by the MS Society, Hull.
