Loved dearly by Michelle, her mum and John. Dear friend of Rose, Dave, Liam, Kate and Daisy the family dog.
Jay's funeral service will take place at Markeaton Crematorium, Round Chapel on Friday 2nd August, 2pm.
No flowers by request, donations in lieu for the Nightingale Macmillan Unit at Royal Derby Hospital may be given at the service. Jay's wish is that, if you are attending, please dress casually.
